This includes education, addiction counseling, employment training, mental health resourcing, safe and affordable housing placement, and public policy advocacy affecting displaced persons.

Access to food is a key social determinant
of health. What's a social determinant of health? According to the CDC, social determinants of health are nonmedical factors that influence health outcomes. It's why partnerships with healthcare facilities like Bayhealth Medical Center are so critical to our work to both alleviate food insecurity and improve health outcomes.

“We really became aware of the need and joined forces during COVID,” said Bayhealth’s Volunteer Service Manager Carrie Hart. She cited a recent experience with two patients, a father and his son who is a person with intellectual disabilities. In planning their discharge, their social worker learned they had no food at home.

“Food is an integral part of health, and many of our patients need a low sodium, low sugar diet,” Hart explained.
