Honor a veteran in your family with a Hometown Heroes banner, proudly displayed right here in Fairview! Whether they served decades ago or recently, this is our way of saying thank you for their service and sacrifice.
You can submit two ways— whichever is easiest for you:
📝 Option 1: Paper Form
Fill out the Hometown Heroes submission form and include a photo of your veteran + a copy of their DD-214 (if available). Payment of $150 (check or money order payable to “Borough of Fairview”) can be dropped off in person or mailed to:

💻 Option 2: Online
Skip the paperwork and complete everything — form, photo upload, and payment — through Cheddar Up. Just scan the QR code on the flyer or use the link below.
