Calling all Carroll County healthcare professionals! We’re looking for a full-time LPN and temporary part-time PT to support our patients, caregivers, and leadership team during an exciting period of growth and accreditation.

We are looking for an expirienced LPN to:
- Assist with patient visits during the accreditation process
- Help teach and support our amazing home health aides
-Coordinate care and ensure quality standards are met
-Answer questions from home health aides and patients with compassion and clarity
This is a permanent position and ideal for an LPN looking to make a direct impact within our community.

We are looking for an expirienced PT to:
• Complete patient evaluations and visits
• Assist with documentation and compliance during accreditation
• Collaborate with our care team to support coordinated patient care
• Serve as a clinical resource when needed
This short-term role is ideal for a PT looking for flexible, part-time work.

As we look back on 2025, our hearts are filled with gratitude for each and every client, family member, caregiver, and community member who chose us this year.

In the past year, our home care agency was honored to:
-Provide 19,356 hours of compassionate care
-Support family caregivers with 1,060 days of guidance and financial assistance through our Structured Family Caregiver Program
-Walk alongside 54 clients and their families
-Employ 46 dedicated caregivers who made this work possible

We’re thrilled to announce that we have officially received our provisional Home Health Agency license from the Indiana Department of Health and are now ready to begin servicing patients. This marks a major milestone as we expand our services to include skilled home health care and pursue Medicare and Medicaid accreditation through the Community Health Accreditation Partner (CHAP).
As part of this process, CHAP will conduct an on-site survey requiring us to demonstrate our ability to provide skilled nursing care in a home setting. During this provisional licensure phase, we are seeking the support of our local medical community through referrals for patients who may benefit from skilled care at home.All care provided will be fully covered by our agency to help us meet accreditation requirements and ensure the highest quality standards.
Our skilled nursing team is prepared to care for patients with a wide range of needs, including:
-Wound care
-Patient and caregiver education for new or worsening symptoms
-Post-operative assessments and follow-up
-Functional decline related to dementia or other diagnoses
-Diabetic education and monitoring (including new monitoring systems)
-Education and monitoring for new oxygen therapy
-Catheter care and maintenance
-Tube feedings and related support
-Monitoring of chronic or serious health conditions

We’re thrilled to announce that we’ve officially hired an OUTSTANDING Registered Nurse and have begun the licensing and accreditation process to become a Home Health Agency!

As part of this process, we are currently seeking individuals who may benefit from no-cost home health services to participate in our initial credentialing phase.

We’re Looking For Individuals Who:
-Are Medicaid recipients (preferred)
-Are Home Bound
-Require assistance with at least one personal care task (e.g., bathing)
OR
-Require skilled nursing care, such as:
-Wound care
-Ostomy care
-Injections
-etc.
-Are not currently receiving services from another home health or home care provider

✨Staff Spotlight-Meet Sarah, our HR Coordinator/Caregiver Float ✨

Growing up in nursing homes alongside her mother, who was a nurse, it was clear early on that a career in the medical field was meant for her. She began her professional journey in mental health, where she dedicated over 13 years before returning to her roots in in-home care.
Sarah has a deep passion for supporting others while respecting their independence, and she finds joy in listening to the life stories her clients share. For her, caregiving is more than a job—it’s a calling.
Outside of work, Sarah enjoys spending time with her husband Dustin and their two children, Kayleigh and Dustin Jr. As a family, they love traveling, cooking out, and making memories together. In her free time, she also enjoys reading, music, and swimming.

Calling on our community for help!

We are ready to make the leap into the skilled nursing industry. We are looking to hire an RN Clinical Supervisor. This is a hands-on position that will require in-person visits with clients in their homes. This position will work predominately with Medicaid and VA clients. Home-care experience and 1+ year supervisory experience required. Preferably, this position will start part-time and quickly grow as caseload grows-I'm happy to make accommodations for the right candidate! We are a small, locally owned company with an "all hands on deck" mentality, our ideal candidate is a team player who isn't afraid to jump in when help is needed!

✨Staff Spotlight! Meet Destini, our weekend caregiver float! ✨

Destini Graduated from Delphi Community High School in 2018 with her CNA license, as she took the CNA course during school, working in various nursing facilities with a staffing agency once she got a year of experience at a local nursing home. She has always had a passion for helping others. In 2021, Destini took a path into psychology and became a Registered behavioral technician, implementing ABA therapy with children on the autism spectrum. Destini works at an ABA center during the week and enjoys providing in home care when needed on the weekends with Elevate Senior Services!
She enjoys in home care due to the personal level of care provided, and being able to do little things for the clients, such as, taking them to their favorite restaurant!

Outside of her professions, Destini enjoys spending time with her husband and 3 year old son. She loves being outdoors, usually running amuck with her toddler. Being a busy gal, She enjoys her moments of self care and bible study as well!

We appreciate Destini’s willingness to go jump in with any client at a moments notice. We are so grateful for her caring heart and expansive skill set!
