If you are unable to fill this out online, please request a fillable copy to be e-mailed and/or arrive 15 minutes prior to your appointment so we can provide you with a paper copy. Services are open to:
AD Service Members, Family Members (18+), Reservists, National Guard, DOD Civilians & Re

tirees.

*Please arrange daycare for small children prior to your visit. Our labs are equipped with sensitive equipment and our services are geared towards adults.

Mark your calendars! Thursday 21 May from 1000-1300 come hang out at our Wellness Fair in the Fort Meade Pavilion. We have over 80 programs/organizations with table set ups with information and activities to promote summertime safety and wellness.

We have food trucks, raffle prizes, and a bunch of community resources excited and ready to help you!
