Warrior Transition Units (WTUs) provide critical support to Soldiers who are expected to require six months of rehabilitative care and complex medical management. The key to WTU success is its Triad of Care, comprised of a primary care manager (usually a physician), nurse case manager, and squad leader who create the familiar environment of a military unit and surround the Soldier and Family with

comprehensive care and support, all focused on the Soldier’s mission—to heal and transition. Warrior Transition Units provide structure, framework and oversight to ensure the number of wounded, ill or injured Soldiers successfully transition back to duty, or successful life as a veteran. The Soldiers of the WTU are dedicated professionals who have sacrificed a great deal to ensure the principles of freedom and liberty are maintained and during Warrior Care Month we honor their sacrifice(s) and join with in the healing and transforming process. At 29 Warrior Transition Units (WTUs) and nine Community-Based Warrior Transition Units (CBWTUs) across the country, wounded, ill, and injured Soldiers have one mission: to heal and prepare for transition. These Soldiers undergo physical rehabilitation, meet with behavioral health therapists, participate in adaptive sports programs, and complete internships and training. Standing with the Soldiers in Transition is the Triad of Care and interdisciplinary team of medical and non-medical professionals who work with the Soldiers and their Families to ensure that they receive the support they deserve.
