Partnership Home Working towards a vibrant community where everyone has a place to call homešŸ  This coordination is called the Continuum of Care (CoC).

A not-for-profit, 501 c(3) organization formed in 1989, Partnership Home is led by a volunteer Board of Directors comprised mainly of homeless service providers. The core of our organization is a coalition of over 100 members including service providers, churches, local government, and individuals. Partnership Home is recognized by the US Department of Housing and Urban Development as the lead dec

ision making and planning body that coordinates Continuum of Care Homeless Assistance Programs including prevention, housing, and supportive services funding for homeless families and individuals in Tarrant and Parker Counties. The CoC planning process creates a strategic system of care to provide homeless people with housing and services appropriate to their needs.

This Supportive Housing Week, we partnered with Presbyterian Night Shelter to provide tours of a local supportive housing community to members of Congress and local elected officials.

At Journey Home Housing, residents receive supportive services and case management to help address important needs, including navigating health care, mental health care, and other challenges.

We appreciate our elected officials for taking the time to tour Journey Home Housing and meet with residents to hear how stable housing has impacted their journeys.
