Frozen and Afraid - Behind the Picture – A National Set Medics Story

This photo comes from Beth, Director of our Reality Medics Division—and if you know Beth, you already know this wasn’t going to be a “normal” job.

Because nothing says “routine production day” quite like this checklist:

- Alaska
- Subzero temperatures
- Cast… fully n**e
- Helicopters as your only transportation
- Oh—and medics carrying fi****ms

Just your standard Tuesday.

This was our first time working on Frozen and Afraid, and from the start it was clear: there would be no “figuring it out as we go.” When your closest hospital is a helicopter ride that depends entirely on weather, you either plan everything… or you plan your problems.

And then there’s the cast. Naked. In Alaska.
Which means frostbite isn’t a “maybe”—it’s basically waiting in the wings like an uninvited guest.

So when you’re handed a situation with this many ways to go sideways, you don’t just send a medic.

You send Beth.

Beth didn’t just prepare—she orchestrated. Every detail was dialed in before anyone set foot on location: flight logistics, extraction plans, cold exposure protocols, wildlife contingencies… all of it. Because once you’re out there, there is no backup plan. There’s just the plan.

And then production added one more twist:
“Hey, also… you’ll need to be armed.”

Because apparently hypothermia and helicopters weren’t enough—let’s throw wildlife into the mix too.

Beth put it best:

“Every variable out there had real consequences. We weren’t just remote—we were completely dependent on aviation. No weather, no movement. No movement, no evacuation.

You had naked participants losing heat fast, terrain that didn’t forgive mistakes, and wildlife that definitely didn’t care about our production schedule.

So every decision had to be made three steps ahead. My job wasn’t just to respond—it was to build a safety net so solid that production could operate like there was a backup plan… even though there wasn’t.”

And that’s the thing about Reality Medics.

We don’t show up hoping nothing happens.
We show up knowing exactly what will happen—and already being ready for it.

This wasn’t just another assignment.
It was one of those moments where everything we train for gets put to the test… in freezing temperatures… surrounded by wildlife… while treating naked people.

Moto-Rescue – Behind the Video

If it involves adventure and medicine, National Set Medics is already there.

Now take that a step further—paramedics on motorcycles.

Think of it like a motorcycle officer… but instead of writing tickets, we’re writing PCR’s. Instead of carrying a gun, we carry oxygen, and ALS gear.

This is one of our lesser-known capabilities: the Moto-Rescue Division, primarily based in Dallas, Texas.

We operate with lightweight, fast-response setups—limited ALS equipment, medications, and M6 oxygen tanks. Some of our bikes are equipped with lights and sirens, allowing us to move quickly and efficiently through crowded or restricted environments.

At large-scale events like marathons and citywide cycling races, our role is simple but critical:
go where no one else can.

We patrol the course, often reaching patients before traditional units can. Sometimes we’re flagged down by participants in distress. Other times, we’re dispatched and threading through crowds, obstacles, and tight access points to get there first.

One standout operation was the Emmitt Smith Gran Fondo—a 100-mile race across North Texas.

We led the lead pack.

With law enforcement controlling major intersections, my role as lead Moto-Rescue was to run point—using lights and sirens to signal our approach and keep on course. Behind me, our Moto-Rescue riders managed smaller intersections and intercepted unauthorized vehicles that drifted onto the course ahead of the cyclists.

At the rear of the lead pack, support trucks with full ALS capability followed, ready to respond to crashes or to escalate care when needed.

It’s a coordinated system built on speed, access, and anticipation.
Because in this environment, seconds matter—and two wheels beat ambulances stuck in traffic every time.

When it comes to adventure medicine, National Set Medics doesn’t just show up.

Behind the Picture – A National Set Medic Story

This is Chris — one of our standout medics.

Chris worked on Naked and Afraid (including one shoot during a hurricane in Florida), along with a number of traditional set medic jobs. But one project in particular stands out: The Chosen, Seasons 1 and 2.

This photo is of him on set of The Chosen.

Before The Chosen became the global phenomenon it is today, I caught wind of the project early — before it even went into production. I invested a few thousand dollars and even offered our services at a discount just to get involved. I wanted to be part of it. Something about it felt different… like it was going to be big.

Turns out, that instinct was right.

As a Christian, it also meant a lot to support something with that kind of purpose behind it.

Chris was part of that journey, providing set medic services on Seasons 1 and 2 through us. Since then, he’s continued to grow in his career and has become a union medic — something we’re incredibly proud of.

As a donor, I was invited to be an extra and ended up as one of the Roman guards in the background.

Don’t bother looking… you won’t find me.

Remember Amanda? We’ve got some fun updates—and a couple of well-earned rewards—for her.

First, Amanda just submitted our 19,000th timecard. That milestone represents an incredible achievement for NSM—19,000 individual medic shifts, totaling a low estimate of 152,000 hours of on-set coverage since we started tracking this data.

Second, when others have called out, Amanda consistently steps up. She’s taken on multiple double shifts for a very high-profile production without hesitation—exactly the kind of reliability every production depends on and appreciates.

To personally thank her, Melissa and I will be sending Amanda one of our limited-edition Challenge Coins along with an official National Set Medics patch.

IT HAPPENED AGAIN.

We’ve once again been contacted by a medic who was not paid—Now I'm officially calling that company out publicly! "The Set Medics.” This medic reached out to us by mistake, thinking we were the responsible party.

Let’s be very clear:

At National Set Medics, we pay our medics—every time—regardless of whether we’ve been paid by production. That’s a standard we’ve upheld for over a decade, without exception.

We’ve absorbed tens of thousands of dollars in losses over the years from productions that failed to pay us, but we have never passed that burden onto our medics. If you work for us, you get paid, Period!

This post serves as both a warning and a reminder to those in our network:

Be careful who you choose to work with.

Your time, your professionalism, and your safety matter to NSM —and you deserve to be paid for it.

To Shaun and those others who are responsible:

Take care of your people. Pay your medics!

If you hadn't been paid let us know and we'll put them on blast or if privately I'll pass legal information to you on a rumored class action lawsuit.

Behind the Picture – A National Set Medics Story

Alright, this one always gets a laugh—for two reasons. First, he’s practically naked. And second, he’s wearing the exact same f***y pack I had on in the “riffle” picture from an earlier story.

I originally took this photo because people constantly asked if the medics on Naked and Afraid had to be naked too. Naturally, we told them, “Yes!”—and followed it up by posting this picture.

The truth? We were actually working a series of bodybuilding competitions at the time. I paid one of the competitors $50 to stage the shot, and he was more than happy to oblige.

Between all the tanning oil and competition prep, getting tape to stick to his back was a challenge—but in the end, it was absolutely worth it.
