And our foundational programs…
Care Coordination for transitions during aging, advance care planning, memory loss, end of life, and Caregiver support!

Along with our amazing volunteers programs providing Rides, Chores, and a matching volunteer program for friend visits!

So grateful to our amazing staff: Christie, Minda, Barb, Jennifer and Julie. And our amazing volunteers… together we do our part toward an age friendly cook county!!

Thank you to all who came out last night to The Splints Benefit Concert for Care Partners at Up Yonder! It was a big success! $3150 given by your generosity!! A HUGE thank you to Carl Solander for imagining and spearheading a benefit for Care Partners!!! And for the Splints and Up Yonder for your talents and hosting!!

Our first year of Intergenerational Pen Pal program wrapped up last week! Our program coordinator, Minda Andrus, did an amazing job of imagining and making it happen in partnership with TJ and Emily, super hero 4th grade teachers at Mountain Elementary School! The capstone of the year of letter writing was a popsicle social at the school where 34 students and elders met each other for the first time in person! It was really extraordinary! Many parents joined the event too! One parent said it was their child’s favorite part of the 4th grade school year!!! We are already looking forward to making the magic happen next year again!! Congratulations to Minda, TJ, Emily and all the Intergenerational volunteers!!! 🫶🏻
