Supportive Care on the Journey of Aging or Serious Illness:

Care Partners of Cook County provides vital support and services to help clients and their families address the transitions of aging, serious illness and end of life with safety, dignity, and confidence. Care Partners works in collaboration with the North Shore Health Care Foundation, Cook County North Shore Hospital, Sawtooth Mountain Clinic and other community service organizations.

Dementia Specialist | Care Coordinator
Do you have a heart for walking alongside people living with memory loss—and the family caregivers who love them? Care Partners is hiring a part-time Dementia Specialist | Care Coordinator to provide compassionate guidance, practical support, resource navigation, caregiver education, and connection for families navigating dementia or mild cognitive impairment.

This flexible role is 20 hours per week and is a meaningful fit for someone with nursing, social work, health education, gerontology, or related experience who believes no family should have to navigate memory loss alone.
