Adaptive aids

Theses are used for many different things such as wipes (2 packets are never enough!), gloves (they are not covered by medicaid.),Nutritional Supplies and many more items!


Minor Home Modifications

Should your loved one need a bathroom remodeled or doors widened in the home where they live this services allows a max of $7500.00 per consumer for life. There are some restrictions call or email should you have any questions!



Counseling and therapies

(includes audiology; speech/language pathology, occupational or physical therapy; dietary services; social work; and psychology)



Dental Treatment

Services are provided by the dentist of your choice, and the max allowed for services is $1000.00 paid directly to the dentist.


Nursing

Services are provided by an RN that will assist you in monitoring the overall care of your loved one, and is available to provide assistance in finding, changing doctors/medical professionals. The RN will also assist in making sure that you understand the proposed treatment that your doctor might order, and help you make informed decisions.


Residential assistance

Supported home living

Used for families with Minor children living in their parents homes, the program pays for a provider to come and assist you in your home to help take care of your child.


Foster/companion care

Used for those who have a consumer who is either over 18 and living with his parents or living with a non-family member

Supervised living

Used for group homes where the consumers do not need 24 hour awake staff.


Residential support

Used for group homes where the consumers do need 24 hour awake staff.


Respite

Used as needed to supplement Supported Home Living when there is an emergency or give the primary care giver a break from their duties.


Day Habilitation

Usually a community center that assist people with disabilities interact with others that are at the same level. They foster socially acceptable behaviors as well as develop independence in the areas of daily living, and just to have some fun!



Supported employment

Is used to help supplement the services that are given through the state to assist you’re loved one in obtaining/maintaining a job in the community.
