The Heart Matters Foster Family Agency

The Heart Matters Foster Family Agency The Heart Matters is dedicated to serving foster youth of the Inland Empire by providing quality foster care, mental health, and transitional housing services.

We care for children between the ages of 0-21 throughout San Bernardino & Riverside Counties. Foster Care is a service provided to ensure that children who must be separated from their families are cared for by nurturing families who are trained, licensed, certified, and approved to meet the child’s needs. The Heart Matters is involved in certifying all our parents. We not only conduct their first

interview but provide all training in-house including First Aid/CPR. Our staff also conducts all assessments and housing inspections to ensure our parents are ready emotionally and physically to foster children. Being involved in this process allows our agency to know our parents on a deeper level and match them with children who require care. Foster Care has a primary goal of providing opportunities for children and families to heal, grow, and develop so that children can be reunited with families who are able to provide safe, nurturing relationships intended to last a lifetime. The Heart Matters is currently recruiting parents and families who are willing to open their hearts and homes to children in need. If you are interested, please call (909) 906-1023 to start the process today! The Heart Matters Mental Health Center provides services to all foster youth within our agency. Services include, but are not limited to, traditional therapy, equine assisted therapy, play therapy, music therapy, and recreational therapy. Our highly trained clinicians provide youth ages 0 to 20 years old, with immersive mental health treatment to assist with their healing from past trauma and mental health challenges. Therapy is provided onsite, and when circumstances permit, within the community or in the home. Services are tailored to meet the needs of the youth through individual, family, and group settings. Our mental health staff and services are sensitive to all cultural values, sexual orientations, and gender identities, and are available in English and Spanish. The Heart Matters Mental Health Center is not currently accepting patients from outside of our agency. The Heart Matters Transitional Housing Program provides guidance and care to current and former foster youth between the ages of 18-21. Youth who are involved in the state foster care system struggle with basic common life skills that most take for granted. When these children turn 18, they enter the community with little to no support. Our Youth Service staff works to ensure that the young adults within this program become productive adult citizens that are educated, employed, and connected to their community. When a young adult is placed in our program, each individual is provided a fully furnished apartment that is preferably located near their current place of employment or school. Each apartment is provided for the youth rent-free along with their utilities and stipends for groceries. Our program also assigns each participant an Evolution Coach, which work tirelessly to make sure that the youth receive the support necessary for them to live successfully on their own. Our staff meets with each young adult weekly and is available 24 hours a day for crisis support. The Heart Matters believes that the extra support these exceptional young adults receive is what separates our program from others and leads to successful outcomes. If you, or someone you know, is in need of Transitional Housing services for foster youth ages 18-20, please contact us at (909) 906-1023.

Every story has a place where it begins.
For many of the youth we serve, healing doesn’t start with a grand moment.
It starts here.
With a safe home.
A caring adult.
Someone willing to say yes.
Because every child deserves a place where hope can begin.

As another school year begins, we couldn’t be more proud of what our community accomplished together. ❤️📚

This past Sunday, our youth spent the day celebrating at the skating rink, receiving brand-new backpacks, school supplies, and name-brand shoes—all because of the incredible generosity of our community.

Every donation, every partnership, every volunteer, and every person who believed in this mission helped ensure our youth walk into the new school year feeling confident, prepared, and deeply cared for. Moments like these are a beautiful reminder that it truly takes a community to make a lasting impact.

To everyone who donated, shared our campaign, partnered with us, or cheered us on—thank you. Your kindness is helping change the lives of youth in our care, one backpack, one pair of shoes, and one act of generosity at a time. 💙

✨ Keep your eyes out for our next community event! Join us for our Open House on Wednesday, September 16th at our Grand Terrace office. We’d love to welcome you, share our mission, and show you how you can become part of The Heart Matters family.

If you’d like to get involved, become a resource parent, volunteer, or simply learn more, send us a DM or email Julian at [email protected]

☀️ Summer with THP! 🌳

Our THP team and youth spent the day soaking up the sunshine and enjoying a summer day at the park filled with activities, laughter, and connection! ❤️

Moments like these are about more than just having fun—they’re about creating space for our youth to connect, build community, and simply enjoy the season together.

A huge thank you to our incredible THP team for continuing to create meaningful experiences and memories for the youth we serve. 🫶🏽

Only 10 wishes remain. 💛👟

Because of this incredible community, we’re just 10 pairs of shoes away from ensuring every youth in our care begins the school year with confidence.
A new pair of shoes may seem small, but to a young person starting a new school year, it can mean walking into the classroom feeling seen, included, and ready for what’s ahead.

Over the next 10 days, we’re inviting 10 people to make a difference by sponsoring one youth’s shoe wish.

Will you be one of them?
If you’d like to purchase a pair of shoes for one of our remaining youth, please send us a DM or email Julian at [email protected] to receive a wish list.

Together, let’s make sure every child starts this school year knowing their community showed up for them.

10 youth. 10 days. 10 opportunities to change a school year. ❤️

Do you remember your first day of school?
Maybe you remember putting on the pair of shoes you’d been dreaming about all summer. Maybe you remember wishing you had them.
For the youth we serve, a new pair of shoes is so much more than a fashion statement. It’s confidence. It’s belonging. It’s walking into a new classroom without worrying about what everyone else is wearing.
That’s why, every year, The Heart Matters provides every youth in our care with a new name-brand pair of shoes, a backpack, and school supplies before the first day of school.
We’re in the final stretch of our Back-to-School campaign, and there’s still time to make a difference.
👟 Sponsor a pair of shoes.
❤️ Help a youth walk into the new school year with confidence.
📩 DM us or email mailto:[email protected] to get involved.

Together, we can make sure every young person starts this school year dripping with confidence, stability, and love.
