Service Coordinator
Early Intervention Program

The mission of CMU is to plan with and support adults and children with mental disabilities, and their families when appropriate, to live full and inclusive lives within their communities. This will be achieved by providing support to access resources, both within the community and within the public systems for mental health, intellectual and developmental disability programs and early intervention services.

Responsibilities:
Early Intervention supports young children and their families to help their child grow and develop. Early Intervention Service Coordination is an ongoing set of activities that assists families/caregivers of infants and toddlers with developmental delays to access Early Intervention services, as well to coordinate and monitor those services.

Supports Coordinators embrace Pa Office of Developmental Program’s Everyday Lives values and principles to support individuals and families.

The role of the Supports Coordinator is to locate, coordinate, and monitor services for individuals which includes activities including:

-Facilitating the participation of individuals, families, and support teams in the development of individuals’ person-centered support plans.

-Locating both community and system supports and services and make referrals to providers on behalf of individuals.

-Coordinating supports and services for the individuals served.

-Monitoring to assure that individuals receive the quality, type, duration, and frequency of supports and services as documented outlined in the individual’s support plan.

-Verifying health and wellbeing of participants through individual monitoring and other contacts with the individual, family, and service providers.

-Identifying and documenting unmet and/or projected support needs with individuals and their families to plan for future funding opportunities.

-Informing individuals of their rights.

-Documenting all services through a statewide, web-based record system.

-Participating in mandatory and ongoing training to enhance skills.



Qualifications

Bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science; or

Associate degree which includes or is supplemented by at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice or other related social science AND two years of relevant experience

PA Child Abuse, PA Criminal History and FBI clearances are required.

Must have vehicle and valid driver’s license.

The Supports Coordination Assistant Clerk Typist 3 provides support to the Intellectual & Developmental Disabilities program to fulfill CMU’s mission and support day to day operations.

The daily activities of Supports Coordination Assistant Clerk-Typist 3 include but are not limited to:

• Proficiency in the use of database systems used by CMU such as the Home and Community Services Information System (HCSIS).
• Proficiency in the use of any other computer-based data systems that are used by CMU and Dauphin County MH/ID Program.
• Data entry of consumer information/records into the Home and Community Services Information System (HCSIS), CMU and Dauphin County MH/ID Program data system and/or any other data systems used by CMU (e.g.: Demographics, ISPs, PUNS, Monitoring Reports, Service Notes, Release of Information etc…).
• Print, copy and distribute designated reports on a regular, periodic basis.
o Use available database applications to determine the appropriate Service/Supports Coordinator to receive records sent by service providers or other sources.
• Receive, review, and distribute records received from service providers.
• Data integrity/reconciliation of information housed within multiple data systems (i.e.: Assuring that information in HCSIS and the local database matches for each individual).
• Print, review and/or analyze reports from HCSIS and/or the local database on a regular, periodic basis as required.
• Summarize data as needed for CMU management staff using established electronic reports from state or local database sources.
• Distribute reports to staff as directed.
• Assist supervisory staff in tracking, preparing and/or analyzing information for review and action for activities such as: IM4Q and NCI compliance, internal chart auditing activities, Incident Management tracking, ISP process related tracking, etc.
• Assists Supervisory staff in maintaining, reviewing and providing reports regarding tracking for initiatives, target categories or other areas as required.
• Assist Supervisory staff by interacting with Supports Coordinator to collect, clarify, correct and/or organize information for HCSIS/local database entry and/or reporting.
• Organizing, maintaining and retrieving records related to provider referrals and related documentation.
• May interact with Dauphin County MH/ID Program and/or service provider staff.
• Perform tasks required for bulk mailings as required.
• Special projects as needed for CMU objectives.
• Other clerical duties as may be required.

Today is the Annual Holiday Turkey Basket Project! CMU Case Managers and Supports & Service Coordinators from our Harrisburg, Elizabethville, Lebanon offices and our partners from Keystone
& Merakey will deliver 275 - 30 lb. boxes of food, eggs, milk, potatoes & a turkey or ham which will feed over 1,000 people in total. CMU supports individuals with mental health, Intellectual and development disabilities and early intervention needs. It’s the best day of the year!
