I'm doing a control study on optimal parameters for monocular and binocular steady-state visual stimulation. I am also comparing traditional signal averaged VEPs versus rapid frequency-domain Fourier analysis.
Under the proper anesthetic conditions, visual cortex (Area 17) sustained responses to steady-state dichoptic flash stimulation are easily recorded.
I believe ... these responses will not only indicate a grossly intact visual pathway, but (based on the temporal tuning characteristics of the retina), such responses can be used to separate magnocellular and parvocellular responses and thus predict if the sub-pathway needed for normal spatial resolution (normal vision) remains intact during the course of high-risk neurovascular or tumor resection surgeries.
In short, this is a hopeful recording technique that can be configured to run on any Cadwell, Medtronic, or XLTec ... and more importantly it may help to reduce the likelihood of intraoperative loss of vision.
This technique is an improvement to the traditional flash transient VEP that has been used for years in the clinic and has been attempted in several other surgical studies with little success.
I actually used a similar technique back in the late 1980s on infants with bilateral latent nystagmus and congenital cataracts with good post-op predictive success.
Friends in IONM, I need some help. I know some of you out there work in the outsourced/contracted IONM setting. And, you are forced to wear the way-too-big paper scrubs, red cap, visitor badge, etc. As some of you know, I'm trying to fight this trend because I think it erodes teamwork and endangers patient safety. I'm giving a very important talk to some decision makers in the surgical world in a couple weeks, and I'm looking for some photos to drive home the point. The more ridiculous you look, the better.
