Healthy Communities Healthy Communities is a Medicaid and CHP+ outreach and case management program.

Healthy Communities (formerly EPSDT) is a community-based outreach program designed to assist families, children, young adults through age 20, and pregnant women to find appropriate services. The Healthy Communities Family Health Coordinator can assist you in working with Medicaid and CHP+ and we can connect you to medical, dental, vision, and behavioral health providers. We will help you make sur

e that your child gets the care he or she needs by letting you know about your child’s Medicaid and CHP+ benefits and by connecting you with other services that are available in your community.
