UBU Synergy Support is looking to recruit a diverse employee base in order to best serve our population! Working as a Community Support Professional you will assist individuals in their own home as you support them to lead the life our clients see for themselves. You will provide support with a variety of home and community activities including support with finances, community engagement, family involvement, health care, household management, and other areas of daily living that the individual served may have difficulty addressing independently.
Requirements:
· Applicants must be 21 years of age and have a High School diploma or GED
· Minimum of 1 year full time or 2080 hours of work experience required; 3 years preferred; education may be considered in lieu of some work experience
· Pass Department of Human Services criminal background check
· Must possess a valid driver’s license and valid auto insurance
· Must be able to travel in all types of weather, willing to travel around the metro area and must have a reliable vehicle
· MUST BE ABLE TO WORK COHESIVELY WITH PEOPLE OF DIFFERENT CULTURAL AND SOCIAL BACKGROUNDS
· Proficient reading, writing and verbal skills in both native language and English
· Experience with Microsoft Windows, Microsoft Word, Email to accurately case note and complete time-sheet
· Must be physically able to climb stairs, walk for extended periods of time and light to moderate lifting for housekeeping tasks
· Must be adaptable to a variety of environments and settings in the community while remaining professional in all types of situations
· Must have the ability to assess physical and emotional needs and be able to respond appropriately
· Ability to multi-task and work under time constraints due to unit-based billing
· Be flexible in order to fulfill service hours as well as assisting in an emergency situation

· Provide therapeutic skill-focused services to support clients as they work towards individual goals
· Alleviate barriers to assist client's integration into the community
· Communicate routinely with client team members, supervisor, co-workers in a team environment with training, delegating and tracking tasks and information
