TAKE ME HOME:
A voluntary, community-centered safety initiative developed to assist individuals who may be at risk of becoming disoriented or unable to return home independently.
This program is designed to support families and caregivers of individuals with:
• Autism Spectrum Disorder
• Alzheimer’s disease
• Dementia
• Cognitive, developmental, or neurological conditions
• Any medical or behavioral characteristic that may impair memory, orientation, or communication
The core mission of Take Me Home is rapid identification, compassionate response, and safe reunification.
* This program reduces response and search times
* Enhances officer preparedness and situational awareness
* Minimizes stress and trauma for vulnerable individuals
* Strengthens trust between law enforcement and the community
How the Program Works:
• Families or caregivers voluntarily complete a confidential signup form with the Jacksboro Police Department.
• The form includes critical identifying and care-related information such as:
o Physical description and recent photograph
o Medical or cognitive conditions
o Communication abilities and behavioral triggers
o Emergency contact and caregiver information
o Known routines, frequent locations, or calming strategies
• Information is securely maintained by the Jacksboro Police Department and used only during emergencies or welfare concerns.
Participation in the Take Me Home Program is entirely voluntary.
• There is no cost to participate
• Individuals may be enrolled or withdrawn at any time
• Information is never shared outside of law enforcement necessity
• The program respects personal dignity, privacy, and autonomy
This initiative exists to support families—not to monitor or restrict individuals.
This initiative reflects the Jacksboro Police Department’s commitment to public safety, inclusion, and responsible stewardship of community trust.
