Ann's Home we give special attention to the living patterns established by our residents prior to coming to the home. This enables each individual to maintain some level of continuity in his or her daily life. We have a care delivery system in place that is called the "Cluster System". The objective of the cluster system is to facilitate better communication, timely resolution of the needs and con
cerns of the residents and to offer a more individualized approach to the resident care. Each floor is divided into 5 sections called "clusters" with 8 resident per cluster. Our Post-Acute Care can provide the rehabilitation services needed for patients following surgery, hospitalizations, injury, or disability. Once the need for acute care has passed, patients who require these types of services must look beyond the hospital for care. Saint Ann's approach to rehabilitation is informed and motivated by the same unique experience and philosophy that guides our long-term care mission. We think of each individual in our holistic care as a "total patient" while focusing on his or her specific rehabilitation needs. Our comprehensive post-acute rehabilitation and skilled nursing services, combined with our unique approach to care, consistently produces successful results for our rehab patients.
