Home for Creative Living
Home for Creative Living was incorporated on October 1, 1975. The original Board of Directors was composed of 12 investors, with Doug Teigen as the chairman of the Board. The facility first opened in September 1977 with the capacity for 45 children in 4 separate units (apartments).
Preferred Residential Lifestyles
In July 1994, Home for Creative Living was downsized to 21 individuals with 24 individuals moving into waivered services. At that time, Preferred Residential Lifestyles-HCL, Inc. was formed by the stockholders of Home for Creative Living. In January 2009 PRL/HCL was formed.
Residential Advantages, Inc.
Windom RAI was incorporated as a private corporation known as Windom Community Residential Facility (CRF) on March 16, 1980 by the 3 original owners, Douglas Teigen, Alvin Nemitz and D. Bill Olson. The name was later changed and Residential Advantages, Inc. (RAI) was incorporated in January 1997.
Luverne RAI was incorporated as a private corporation on May 5, 1980 by the original owners, Douglas Teigen and D. Bill Olson. In May 1994, Luverne RAI was converted to a six-bed facility for persons over age 18.
New Ulm RAI was incorporated as a private corporation on January 19, 1982 by the owners, Douglas Teigen and D. Bill Olson. This facility was changed to a waiver site in New Ulm in April 17, 2007.
Habilitative Services, Inc. (HSI)
Habilitative Services, Inc. was incorporated on April 30, 1984, by the owners, Douglas A. Teigen and DonaeBill Olson. Their partnership continued until Doug’s death in March 2010. With Doug’s passing Bill is the sole owner of HSI. The administrative office is located at 220 Milwaukee Street, Suite 2, Lakefield, Minnesota. Satellite offices are located in Luverne, Marshall, Windom, New Ulm, Mankato, Worthington, Fairmont and Moorhead.
Habilitative Services, Inc. (HSI) is a professional agency serving individuals with impairments that limit their domestic, employment, educational or leisure opportunities. At HSI, we provide value-based services to help individuals achieve their greatest potential. We provide a variety of direct support and consulting services. Our Health Services Department provides internal nurse consulting services, personal care assistant services and respite care. As a licensed Class F Home Care Agency we provide elderly housing with services at Doman-Rose Place and Rosewood Villa in Lakefield, MN. We are licensed as a 245D provider by the Minnesota Department of Human Services (DHS) to provide supported living services under the MRRC waiver program. We are also licensed to provide semi-independent living services. We provide residential support and services to persons under the CAC, CADI, and TBI waiver programs. We provide the administrative and management services for ICF/MR homes that are licensed by the Minnesota Department of Health and DHS. Our Social Services Department provides Guardianship/Conservatorship services and contracted Case Management services. Habilitative Services is involved in the management of all three corporations.
Partnership with The Mentor Network
On September 29th, 2017 The Mentor Network (TMN) successfully completed the acquisition of HSI, PRL-HCL, and RAI.
TMN has deep roots in Minnesota through its REM Minnesota operations, which this year are celebrating 50 years as a leading provider of community-based services for individuals with intellectual and developmental disabilities. In more recent years, REM, which serves 59 of Minnesota’s 87 counties, has successfully expanded into a range of specialty services not currently offered by HSI, including brain injury, mental health and autism services.
REM and HSI overlap in 12 counties, and only in seven counties for residential services that represent the vast majority of each organization’s revenue. TMN management believes this may allow HSI to leverage REM’s specialty-service expertise and expand its offerings in the years ahead.
