We are deeply grateful to The Southpole Foundation for supporting the expansion of Family Touch’s senior wellness, dementia response, and bilingual health navigation services.
The Southpole Foundation is committed to strengthening communities and supporting underserved populations across New York and New Jersey. This mission aligns closely with Family Touch’s commitment to reducing disparities and expanding equitable access to care.
With this support, Family Touch will expand senior services in partnership with Bergen Community College. This work will include senior wellness and dementia response training led by Certified Dementia Practitioners and a Registered Nurse, chronic disease management courses, and bilingual navigation through our Integrated Health and Mental Health Access Center.
For many older adults and caregivers, language barriers, isolation, and complicated systems make it difficult to access the support they need. This work brings care closer to the community and makes support more accessible for the seniors and families who need it most.
