Please join me in supporting CCHAP so we can better serve our clients in Lake Mwndocino and Sonoma Counties! Your donation makes a world of difference in helping to supply affordable life alert safety systems, disaster preparedness kits, stocking a food bank or covering medical supplies. Your donation also allows us to continue to provide free HIV and Hep C testing.

If you cannot make a donation. Please help us by sharing our Go Fund Me with your friends.

In 1988 World AIDS Day was established on December 1st and is observed internationally. In the United States, 1.1 million people.were living with HIV. 15 percent or one in seven are not aware they have been infected according to HIV.GOV in 2018.

CCHAP would like to take a moment and remember all of the friends and family that have lost their lives to HIV/AIDS. They are always with us in spirit. They may be gone but are not forgotten.

We also would like to take a moment and appreciate the opportunities we have to serve our clients. Our team has thoroughly enjoyed getting to know you and lending a hand when needed.

It is in honor of our client, past and present, that CCHAP will continue its efforts to raising awareness about the risk of transmission for HIV and provide free HIV testing in the community as well as case management services to help our clients maintain their highest level of independence and health.
