Having everyone apply helps Manistique Area Schools receive vital Federal funding.

(and then after the agreements, select "Education Benefits Form")

You may need the following to fill out the application:

Student/Child Details: Names and basic information for all children attending school.

Household Income: The amount, frequency (weekly, monthly, etc.), and source of current income for every household member (such as earnings, welfare, pensions, or child support).

Assistance Case Numbers: If applicable, an active case number or identifier for programs like SNAP (Supplemental Nutrition Assistance Program), TANF (Temporary Assistance for Needy Families), or FDPIR (Food Distribution Program on Indian Reservations).

Social Security Number: The last four digits of the Social Security number of the adult household member signing the form (not required if you list an assistance case number or check the box indicating the signing adult has no Social Security number).

Fall Sports begin August 10th, 2026! The MHSAA requires each student athlete have a current sport physical on file to participate. Call the SMH Rural Health Clinic today 906-341-2153 to make your appointment (sport physicals only cost $10 at SMH) and can be scheduled thru RediCare
