RankOne forms and yearly physical:
Starting in 2026, every student in grades 7-12 will need to fill two eligibility forms out each year. Those forms are labeled
* OSSAA Eligibility form 1
* OSSAA Eligibility form 2
Each student should have all sections of the RankOne forms completed (will show each section green and approved) before they will be allowed to compete.
Returning students may login and complete forms now.
Each year, students in Band and Athletics are required to get a new physical.
Athletes grades 9-12 need to have these completed by July 1.
Athletes grades 7-8 need to have these completed by August 1.

The physical form needs to be turned in to the Athletic Department Office at Brumley Gymnasium. Student and Parent/Guardian signatures are required on the hard copy of the physical form.
To log into RankOne, the students ID number will be needed.(contact the athletic department office or students attending school to get the student ID number). New students to the district (Before July 1) will need to come to the athletic department office and be assigned a temporary ID number. Please complete these forms ASAP and be cleared to go!
