No fee for those who do not have insurance. STDs- Testing is provided for s*xually transmitted diseases available to all ages. Vital Records- Birth and death certificates available for Arkansas occurrences. Birth certificates : $12 for first copy - $10 for each additional copy. Death certificates: $10 for first copy - $8 for each additional copy. Water testing: Well Water testing kits available. The fee to test is $17. All visits are kept strictly confidential.
