Attention Student-Athletes for the 2023-2024 School Year...
The Monessen Athletic Department will have Sports Physicals for the 2023-2024 school year on Monday, July 31, 2023, from 8:00 AM to 2:00 PM at Monessen High School on a first come first serve basis. In order to be placed on the list student-athletes must complete the following steps:
1. PIAA Physical Forms were passed out in school on Thursday, May 25, 2023. Student-Athletes must take these forms home and have their parent/guardian fill out all information on all pages, in its entirety. Please make sure the parent/guardian signature and student-athlete signature as well as all medical information is provided on each page.
2. Student-Athletes must return the PIAA Physical Form to Ms. Naccarato. Ms. Naccarato will review the PIAA Physical Form and if everything is completed on the form, the student-athlete will be given a time slot for July 31.
3. Ms. Naccarato will call the parent/guardian with the time slot over the summer.
4. The PIAA Physical is good for one school year once the doctor signs off on it and clears the student-athlete.
5. The only requirement for the PIAA Physical to be good for the 2023-2024 school year is that the physical has to be completed after June 1, 2023. Any physical completed before this date will not be approved for the 2023-2024 school year.
6. Any student-athlete who does not get a PIAA Physical here on July 31 must get the physical on their own with their P*P or at MedExpress. The PIAA Physical Form must be completed by the P*P or MedExpress in order for the student-athlete to be approved to participate.
