Description of our programs & services: Case Management Program (This service is offered in English or Spanish): Together, the client and case manager develop a care plan (following a biopsychosocial model) with goals that, when met, will improve the client’s quality of life. Water Aerobics Program: This program is offered free at the Monterey Sports Center for our clients living with MS or PD and it allows for an accompanying family member. The program is led by an in-water trainer who directs clients and provides a comfortable environment for stimulating exercise and camaraderie. Support Groups: CCQLP currently hosts three monthly support groups in Monterey and Santa Cruz County. Equipment Reuse Program: CCQLP can provide essential mobility equipment (for loan) to individuals who need a mobility device to get around while they wait for a prescribed device. Equipment that may be available through CCQLP’s program: canes, walkers, shower chairs/benches, toilet seats, manual wheelchairs, power chairs, and scooters. Educational Events and Workshops: CCQLP hosts frequent MS and PD related educational events on overcoming disease-related challenges, preventative care practices, and symptom management. We also co-host and help promote pharmaceutical sponsored events that include a healthcare specialist speaker.
