Healthy Ottawa is currently conducting the Community Health Needs Assessment. It’s done every three years and looks at the health of Ottawa County community members. One tool used in the assessment is the Behavioral Risk Factor Survey, which is happening throughout September. This community-wide telephone survey is one of the most important ways we gather information about the health of our community.

If you get a call from a 616 area code (it may show as "Wilkins Research"), please take 20–30 minutes to share your voice! The survey is anonymous, and participation is optional[AC2.1].

Your caller ID will show a 616 area code, sometimes listed as "Wilkins Research," the research firm making the calls on our behalf.

Missed the call? No worries — they may leave a voicemail with a callback number so you can complete the survey whenever it's convenient.

The survey only asks health-related questions. We'll never ask for your Social Security number, bank or credit card info, or any payment, and all responses are anonymous. Participation is optional.

Community Health Needs Assessment Community Survey

Every three years, multiple community partners conduct the Community Health Needs Assessment (CHNA). This helps community service providers and health systems along the Lakeshore identify issues that impact our community and opportunities to improve the quality of life where we live.
The survey will be open until August 20, 2026.
