Community Health Needs Assessment Community Survey

Every three years, multiple community partners conduct the Community Health Needs Assessment (CHNA). This helps community service providers and health systems along the Lakeshore identify issues that impact our community and opportunities to improve the quality of life where we live.
The survey will be open until August 20, 2026.
