Healthy Nevada seeks to define a rural public health model that reduces historically poor health outcomes by addressing social determinants of health through the leveraging of community collaboration and partnerships as the foundation for individual, institutional and community wellness. Our action involves the developmental and implementation of programs that are innovative, integrated and respon

sive to the health needs of rural populations. We will develop and demonstrate replicable models of integrative care using a multi-organizational approach and evidence-based strategies.
