💙 Living with dementia can be challenging, but support is available.

The GUIDE Model (Guiding an Improved Dementia Experience) is a no-cost Medicare program designed to help individuals living with dementia and their caregivers access coordinated care, support services, and resources that improve quality of life.

The GUIDE Model can help:
✔ Improve quality of life for people living with dementia
✔ Reduce caregiver stress
✔ Provide caregiver training and support
✔ Coordinate care and services
✔ Reduce unnecessary hospital visits

You may qualify if you:
• Have a dementia diagnosis
• Are enrolled in Medicare Parts A & B
• Are not enrolled in Medicare Advantage, PACE, or hospice
• Do not live in a long-term nursing home

💙 Another Older Americans Month Highlight 💙

This May, we celebrated more than birthdays, we celebrated the lives, stories, and legacies of the older adults who make our communities stronger.

At 107, Ms. Everlean is a wonderful example of resilience, kindness, and grace. As a mother, friend, and cherished member of her community, she continues to bring joy to those around her.

We were honored to celebrate this incredible milestone and grateful to the students and staff of Nansemond Parkway Elementary School for helping make her birthday extra special with thoughtful cards and gifts.

Moments like these remind us of the power of connection across generations and the importance of honoring the older adults who have helped shape our communities.
