🌱 Transition-Aged Youth (TAY) Services
Cadence Care Network’s Transition-Aged Youth (TAY) services provide intensive home- and community-based treatment to youth in Northeast Ohio between ages 14–21 who are transitioning into adulthood.

Our goal is to assist youth in successfully transitioning into adulthood by enhancing independence, responsibility, and community integration.

📌 Services include:
• Comprehensive assessment and individualized treatment planning
• Life skills development
• Case management
• Mental health counseling
• Assistance accessing housing, employment, and education resources

📍 Serving Columbiana, Mahoning, and Trumbull counties.

We’re excited to celebrate our very own COO-Columbus, Sarah Steuer, for being recognized as one of Central Ohio’s New + Notable Leaders in Nonprofits in the 2026 Citywide Charity Guide.

Sarah’s leadership has always been grounded in one simple idea: do what’s best for the community, even when it requires courage and change.

From leading Youth Advocate Services for nearly a decade to helping guide its transition into Cadence Care Network, Sarah has remained focused on expanding access to care, strengthening support for families, and investing in the people who make this work possible.

Because of leadership like hers, our Columbus community has seen expanded Early Intervention services, new behavioral health support for young children, and continued growth in programs designed to meet families where they are.

We are grateful for Sarah’s steady leadership, compassion, and commitment to building a stronger future for children and families across Central Ohio.
