❌Common Mistake:
Completing Your Documents But Never Sharing Them

Did you know that completing a Healthcare Advance Directive and then storing it somewhere no one can find it is just as dangerous as not having one at all?

Your doctor, healthcare surrogate, attorney, and close family members all need to know these documents exist and where they are located. Florida also recommends carrying a wallet card noting the location of your Advance Directive at all times.

Florida Now Recognizes Four Types of Healthcare Advance Directives?

They are:
- a Living Will
- a Health Care Surrogate Designation
- a Do Not Resuscitate (DNR) Order
- an Organ and Tissue Donation designation.

You can complete one, several, or all four, depending on what best reflects your personal wishes and situation.
