Community Care Coordinator - Rehab Ctr & YMCA The Rehabilitation Center and Olean YMCA joined together to create the first Community Care Coordinator, housed within the Olean YMCA.

Our Coordinator will help individuals in improving their Social, medical & mental health. Life can be difficult at times and we have all needed assistance and/or guidance. Sometimes people are afraid or embarrassed to seek help or they don't know where to go. Our community is blessed with someone available to give you a helping hand. Our Care Coordinator can assist individuals with:
Establishing a

nd obtaining their personal goals
Maintain continuity of care in all areas of their lives (Physical, Mental, Social). She can assist you in learning about our community activities that will enhance your life. Whatever your need, if she can't help you, she will find someone who can. . Our Coordinator is located in the Olean YMCA, you DO NOT have to be a member of the YMCA to access her assistance, but if you would like to be a part of the YMCA Community, she can help you through the process necessary.
