📍Now hiring: Supervising Case Manager / YOFA
💵Salary: $20.65 – $30.97 per hour
📝Position Summary
The Supervising Case Manager/YOFA oversees case management and housekeeping staff, monitors service contracts and vendors, manages reporting and data collection requirements, provides direct case management services, and assists with budget oversight and program administration.
🫶🏻Key Responsibilities
-Supervise case management and housekeeping staff.
Monitor vendor contracts, services, and program compliance.
-Prepare reports and assist with NYSOFA updates, grants, and planning.
-Conduct client assessments, care planning, home visits, referrals, and database entry.
-Provide information on long-term care options and assist with applications.
-Monitor program budgets, process invoices, prepare subcontractor contracts, and coordinate budget modifications.
✅Qualifications
-Bachelor’s degree in Human Services, Social Work, Psychology, or a related field.
-3–5 years of related experience.
-Experience with case management, advocacy, community resources, databases, and virtual platforms.
-Strong organizational, communication, computer, and problem-solving skills.
-Ability to work with diverse populations and exercise independent judgment.
📋Requirements
-Valid driver's license and reliable transportation for regular travel throughout the service area.
-Successful background check.
