• Assuring quality of services
• Assuring your satisfaction with services and supports

Advocacy
• Working with providers to assure you are getting what you want and need
• Helping you to speak up for yourself in realizing your dreams and goals
• Conflict resolution

Individualized Life Planning
• Planning for the future
• Helping you to reach your goals through ongoing, person-centered life planning

Assistance accessing community resources and governmental benefits
• Assisting in applying for: HUD, HEAP, Foodstamps etc.
• Assisting in applying to community resource agencies

Crisis Intervention
• Access to a Service Coordinator 24 hours a day 365 days per year via an emergency on-call system

Service Coordinators assist individuals with disabilities and the elderly to live as independently as possible in the community by coordinating and monitoring community based services. Caseloads are capped at 25 individuals. We offer site based and remote work opportunities as well as flexible schedule, health, dental and eye insurance, mileage, paid time off and retirement. You must have at minimum a bachelors degree and 3 years experience.

There are 2 positions to fill: 1 in the greater Glens Falls and Saratoga Area.

1 In the Albany area.

POSITION QUALIFICATIONS:

An ILST must be a:

Licensed Occupational Therapist;
Licensed Clinical social worker
Licensed master social worker
Certified teacher of students with disabilities
Certified rehabilitation counselor
Licensed Physical Therapist;
Licensed Speech-Language Pathologist;
Registered Professional Nurse;
Master of Social Work; or
Master or Doctor of Psychology.
These individuals must have, at a minimum, one (1) year of experience completing assessments of ADL’s/IADL’s functionally based assessments, developing a comprehensive treatment plan and teaching individuals with Traumatic Brain Injuries to be more functionally independent; OR

(B) An individual with a Bachelor’s degree and 3 years of experience completing assessments of ADL’s/IADL’s, developing a comprehensive treatment plan and teaching individuals with Traumatic Brain Injuries to be more functionally independent; OR

(C) An individual with an Associate’s degree and 5 years of similar experience completing assessments of ADL’s/IADL’s functionally based assessments, developing a comprehensive treatment plan and teaching individuals with Traumatic Brain Injuries to be more functionally independent.

Several months ago, Support-Link's Structured Day Program worked on a birdhouse project. After pictures of some of them had been posted, there was a request on whether there were any for sale. well, we actually had some extra ones, so our program participants painted some more which are available for sale. We are posting four today on our site, and will soon look at posting more. If you are interested, you may contact us directly through this page. In the future, we will have more posts and will eventually use the Queensbury Garage Sale page. Let us know and take a look! You can sport a great-looking & colorful home for your yardbirds this spring! (There is a front view and side view for all four posted birdhouses)

Support-Link's Structured Day Program is contracted with the Care Support Initiative to be able to provide hours of service for individuals with whom they serve. One of the regularly scheduled programs they offer is a "Memory Cafe". They disburse information and usually have a community affiliated presentation, and of course, some refreshments.
