Reports can be made 24 hours a day, 7 days a week, 365 days per year. Please provide as much information as possible to help us identify the child, his or her location, and the nature of the allegation.

Join us Saturday at the Portage County Health District for their clinic. We will be there with a table of information and resources.

Don't miss your opportunity for a FREE sports physical or work permit physical this Saturday, June 13th, at Portage County Health District! To schedule your appointment please call 330-296-9919 EXT 135.

AxessPointe Community Health Centers mobile dental unit will be on site. Cost is determined based on a sliding-fee scale. To make an appointment or for questions please call 330-673-1016 EXT 2.

Additional resources and services will be available including vaccines and vital testing. If you plan to receive a vaccine, please bring your insurance card.

Come dressed and ready for the exam! We recommend tank tops, t-shirts, shorts, and easy to remove shoes.

Students with a serious pre-existing medical condition should schedule a physical with their primary care physician.

Portage County Job and Family Services is excited to announce the launch of the 2026 Back-to-School Clothing Voucher Program. This Prevention, Retention and Contingency service provides $175 vouchers per child to be used to purchase school clothing and shoes. Upon approval of application, the vouchers can be provided to eligible school-aged children (Kindergarten-grade 12) who are in receipt of SNAP or TANF benefits. Applications will be available from June 22 - July 31
