PSA 2 Long-Term Care Ombudsman Program Advocates for over 3000 residents residing in long term care facilities in Lassen, Modoc, Siskiyou, Shasta, and Trinity Counties

The PSA 2 AAA Long-Term Care Ombudsman Program is mandated by the federal Older American's Act and the Older Californian's Act, to investigate and resolve complaints regarding care in long-term care facilities. Residents living in long-term care facilities have certain rights that are guaranteed by federal and state regulations. The Ombudsman Program advocates to protect and promote these rights a

nd to empower residents to be self-advocates. Through public education, the Ombudsman Program seeks to promote a better understanding of the long-term care system of care. In sum, the Ombudsman Program provides information, advocacy, and assistance to resolve concerns regarding the quality of care in long-term care facilities.

Every resident has the right to be free from unnecessary restraints. These practices can impact dignity, independence, and overall well-being.

Understanding what restraints look like is the first step in protecting resident rights. The Long-Term Care Ombudsman program advocates for residents, works to address concerns, and helps ensure care is safe, respectful, and person-centered.

Slips, Trips & Falls in Long-Term Care Are NOT “Just Accidents.”

Falls are one of the most common and preventable causes of injury in long-term care facilities.

Wet floors. Poor lighting. Cluttered walkways. Lack of assistance. Medications that cause dizziness. These are not small issues, they put lives at risk.

A single fall can lead to broken bones, head injuries, loss of independence, and fear that changes everything. We must do better.

Keep spaces safe. Respond quickly. Provide proper support. Protect those who depend on us.

Don’t be afraid to tell.

Abuse in long-term care doesn’t always leave visible marks, but it leaves real harm. Silence protects the wrong people. Speaking up protects those who can’t always speak for themselves.

If something feels wrong, it probably is. Trust that instinct. Whether you’re a resident, a family member, or a caregiver, your voice matters.

Report concerns. Ask questions. Check in often. Advocate fiercely.
