We are enjoying connecting with community members, sharing information about Enhanced Care Management (ECM) services, and highlighting housing resources available to support individuals and families throughout our community.

Events like the Redding Health Expo provide valuable opportunities to build relationships, increase awareness of available services, and help connect people with the care and support they need to thrive.

We are excited to be part of Redding's 10th Annual Health Expo and invite our community to stop by our table to learn more about the services we provide throughout Shasta County and surrounding areas. You will meet Lead Care Manager Oksana Knapp who has been serving Shasta County for over a year!

At Sowing Seeds of Health, our mission is to improve health outcomes, reduce health disparities, and connect individuals and families to the support they need to thrive.

💚 Learn about Enhanced Care Management (ECM)
Our ECM program provides comprehensive care coordination for individuals facing complex medical, behavioral health, and social needs. Lead Care Managers work one-on-one with eligible Partnership HealthPlan members to help navigate healthcare systems, connect to community resources, coordinate appointments, address social determinants of health, and support long-term wellness goals.

🏠 Discover Our Community Supports Housing Services
We also provide Housing Transition Navigation Services, Housing Deposits, and Housing Tenancy & Sustaining Services for individuals experiencing homelessness or housing instability. These services help individuals secure safe, stable housing and maintain their housing while addressing barriers that impact health and well-being.

Access to healthcare, housing, food, transportation, and supportive services plays a critical role in overall health. By helping individuals overcome barriers and connect to essential resources, we are working to create healthier communities and reduce preventable health disparities throughout our region.

June- Men’s Mental Health Awareness Month

June is men's mental health awareness month and it's an opportunity to recognize something that often goes unspoken.


To the men who show up every day for their families friends coworkers and communities- this month is for you.


For generations many men have been taught that strength means staying silent carrying every burden alone and pushing through pain without asking for help but true strength is not found in suffering in silence true strength is found in honesty vulnerability and the courage to reach out when life becomes overwhelming.


Mental health affects everyone. Stress, anxiety, depression, grief ,loneliness, and self-doubt do not discriminate based on gender. Yet too many men feel pressured to hide their struggles out of fear of judgment or appearing weak.


This month we challenge that idea.. We celebrate the men who speak openly about their mental health. We support the men who are still finding the words. We remember those who fought battles no one else could see. And we encourage every man to know that seeking help is not a sign of failure- it is a sign of self-respect and courage.


To every father son brother husband friend mentor and colleague your well-being matters, your feelings matter, your life matters. May this month be a reminder that you do not have to carry everything alone. There is strength in connection, power in conversation, and hope in reaching out.
Let June be more than awareness. Let it be a commitment- to listen more, support one another, and create a world where men feel just as comfortable talking about their mental health as they do talking about their physical health.


Because every man deserves to be heard supported and valued- not just for what he provides, but for who he is.

Sowing Seeds Health is proud to offer Enhanced Care Management (ECM) services to individuals and families in our community — and we’re currently accepting new clients!

✨ What is ECM?
ECM is a FREE service for members with Partnership HealthPlan that provides personalized, whole-person support to help you navigate healthcare, housing, and life challenges.

💚 Who We Serve (Populations of Focus):
• Individuals experiencing homelessness
• Those with serious mental health or substance use needs
• Members recently released from incarceration
• Individuals at risk of long-term care placement
• Those transitioning out of skilled nursing facilities
• Children, adults, and seniors — we support all ages!

🤝 How We Help:
• Care coordination & advocacy
• Access to medical, mental health, and social services
• Housing support & resources
• Goal planning for stability and independence

🌱 What Are Populations of Focus — and Why Do They Matter? 🌱

In our community, certain groups face greater barriers when trying to access consistent, quality healthcare. These groups are known as Populations of Focus — individuals who are at higher risk for poor health outcomes due to complex medical, behavioral, or social needs. This can include people experiencing homelessness, those with chronic health conditions, high hospital utilization, serious mental illness, substance use challenges, or individuals transitioning out of institutions/incarceration.

Supporting these populations doesn’t just help individuals — it improves overall community health. When people receive the right care, resources, and support, we see fewer preventable hospital visits, better stability, and stronger long-term wellness.

✨ How Enhanced Care Management (ECM) Helps
Enhanced Care Management is a person-centered program designed to connect people to the care and support they need most. ECM focuses on breaking down barriers, improving care coordination, and addressing both medical and social factors that impact health.

✨ What Lead Care Managers Do
Our Lead Care Managers work directly with clients to:
✔️ Conduct regular check-ins to understand needs and goals
✔️ Coordinate medical, behavioral health, and community services
✔️ Support with housing, transportation, food resources, and referrals
✔️ Advocate for individuals during appointments or system navigation
✔️ Ensure follow-through on care plans and help reduce gaps in care

By guiding each person with compassion and consistency, Lead Care Managers help clients feel supported, empowered, and connected—leading to improved health outcomes and better quality of life.
