Rockford Ready is an initiative led by the City of Rockford Health and Human Services Department that is dedicated to improving the health of all Rockford residents by providing health education, connecting residents to health resources, and strengthening relationships between health care providers and residents. In partnership with the University of Illinois Chicago and influential leaders in Roc
kford, we are an organization for the community, by the community. Our partners include Rockford community leaders who have a deep understanding of, and strong relationships with, Rockford residents. Through these connections we aim to provide resources that meet the unique needs of Rockford residents.
