Greetings East of the River Community Collaborative families, agencies and service providers and guests!

Our next meeting is scheduled for Thursday, October 19, 2017 from 9:30 am - 11 am.

It will be our first meeting combined with the members of the Route 2 Community Collaborative and will be a Meet & Greet and Provider Fair. More attendees will be agency personnel, but families are most definitely invited and encouraged to attend.

It will take place at our regular site, but in the downstairs auditorium called Cooper Hall.

Tables will be provided for you to set up a small display of your agency or organizations hand-outs and information.

We thank Kelly Waterhouse of the East Hartford Youth Services, Lisa Palazzo from Beacon Health Options and Teri Rogers and Laura Corliss from the Vernon Community Network (VCN) in helping organize this event.

Our following meeting will be November 16, 2017 and will be hosted by:

Rafael Rodriguez Cruz – Greater Hartford Legal Aid – Staff Attorney
Rafael Rodriguez Cruz has been a staff attorney at GHLA since 1997. He focuses on children’s advocacy, representing clients in the areas of education and Social Security/SSI benefits. He also conducts community outreach, currently working in the towns of Bristol and East Hartford. He is fully bilingual (English/Spanish). Rafael is one of the co-chairs of the East Hartford Community Partners Group, a coalition of human services agencies in the city of East Hartford.

Our East of the River Community Collaborative is one of 25 in the State of CT. We serve the following towns of Bolton, Ellington, Manchester, South Windsor, Tolland and Vernon, CT.
We meet monthly as a collaborative to educate, train, and promote the benefits of the values and principles of System of Care and the Wraparound Process. These include:
• Family Driven and Youth Guided
• Strength Based
• Cultural and Linguistic Competence
• Individualized, Flexible and Community Based Approach to services and support
• Services and Support Provided in Least Restrictive and Most Normative Environment
• Adequate Availability and Access to Broad array of Effective Services and Support
• Evidence and Science Informed Clinical Interventions, Services and Supports
• Health and Wellness Promotion
• Performance and Outcome Based services and support
Our vision as a regional collaborative is that all children, youth, and families are able and capable of achieving optimal levels of functioning at home, in the community, at school and/or work. WrapCT developed out of a decade of System of Care improvements, including on-going training and coaching through Care Coordination and the Child and Family Team Wraparound process.

Bonnie Roswig of the Center for Children's Advocacy. She has a wealth of experience advocating for those who most often can't. She is the Senior Staff Attorney for the Center’s Medical-Legal Partnership Project, with an office located at Connecticut Children’s Medical Center. Prior to joining the Center, Bonnie worked as a legal aid attorney in Georgia, and was a supervisory attorney at Statewide Legal Services in Middletown, CT. Bonnie is an Adjunct Professor in the Legal Process Program at the UConn School of Law. She is a graduate of Bennington College and Antioch School of Law.

Bonnie Roswig was selected as a 2011 Health Leadership Fellow by the Connecticut Health Foundation. About 20 Fellows are selected each year through a competitive process for a one-year knowledge and skill-building program designed to create leaders who represent a variety of public and private sectors in public policy, health practice, health care administration, community, law, business and commerce, advocacy, academia and other related fields. In addition to education sessions and training, Fellows participate in a project of their choosing which demonstrates leadership ability and influence to take collective actions that eliminate racial and ethnic health disparities.


Allison Matthews-Wilson is the Clinical Care Coordinator and Facilitator of the North Central Care Coordination Collaborative(NCCCC) and Katherine Ramirez Senior Care Coordinator and Technical Advisor to the NCCCC The purpose of the NCCCC is to serve families and child health care providers in the greater Hartford area in a holistic and efficient manner by identifying and maximizing the full range of resources available and supporting care coordinators in obtaining the care and services needed by children and their families.

In addressing the needs of families, the Collaborative also seeks to understand health and human service delivery systems in order to promote wellness, support the medical home, and assist families in negotiating these systems as well as to document the gaps and barriers that families confront when in need of assistance.
