Day Haven Adult Day Services Social Model Program Supporting Elders, Caregivers, and Life We offer flexible scheduling and full or part time enrollment.

Our social model adult day services program provides day-time care and supervision, a full range of therapeutic activities, and socialization opportunities for frail older adults and individuals with Alzheimer’s or a related dementia. Since 1984, Day Haven has been a pioneer in developing services to support elders in their desire to remain at home for as long as possible. Our therapeutic activiti

Our commitment to caring ensures all participants are treated with the dignity and respect they deserve. Our knowledgeable staff including a social worker, recreation professionals and paraprofessionals can help you and your loved one make the transition to adult day services.

Day Haven Port Jeff would like to thank Sarah M, for sharing her beautiful music with us every Thursday. Sarah uses her angelic voice to lead sing-a-longs to beautiful songs like “Somewhere Over the Rainbow” , “Hey Jude” and “You Belong To Me”.
