If you are an Umpqua Health Alliance member age 18 or older, you may be able to earn Sherm's Thunderbird Market gift cards through the Prevention First Incentive Program.

Eligible services include annual physicals, dental visits, flu shots, mammograms, cholesterol tests and more. Complete qualifying services by Dec. 31 and earn up to $75 in gift cards.
