Job description
Independent Living Skills/In Home Support Specialist

In Home Supports with training is direct training from a staff person to address the identified skill development needs of a person in the areas of:

Communication skills
Community living and mobility
Interpersonal skills
Reduction/elimination of challenging behavior
Self care
Sensory/motor development involved in acquiring functional skills.
Examples of job duties:
Teaching people with disabilities how to budget their money
Teaching how to fill out paperwork for county benefits
Teaching resources in their community
Teaching how to cook
Teaching how to ride the bus system

Requirements for position:

Must have experience working with people with disabilities (1 year)
Must be able to pass a DHS screening
Must have active driver's license with reliable transportation and be willing to transport clients in your own vehicle
Must be comfortable driving throughout the metro area
Must have Car Insurance and be able to provide proof of insurance
Comprehensive benefits package
This position will be provided in the person's home or community setting. Paid cell phone reimbursement. Mileage reimbursement. Flexible schedule. No weekends. No Holidays.

Salary: $18.50 - $22.00 per hour

Believe Again Home Care is preparing for our annual homeless outreach event!

We are asking for donations in the form of hats, mittens, hand warmers, dry shampoo, deodorant, and personal wipes.

We will have two teams going out at 1pm on November 10th. One in St. Paul and one in Minneapolis.

Believe Again Home Care is looking for Housing Case Managers to join our growing team. BAHC offers a comprehensive benefits package, paid mileage, cell phone reimbursement and more!

The Housing Case Manager is responsible for:

Developing a housing transition plan*
Supporting the person in applying for benefits to afford their housing, including helping the person determine which benefits they may be eligible for*
Assisting the person with the housing search and application process*
Assisting the person with tenant screening and housing assessments*
Providing transportation in your own vehicle with the person receiving services present and discussing housing related issues
Helping the person understand and develop a budget
Helping the person understand and negotiate a lease
Helping the person meet and build a relationship with a prospective landlord
Promoting/supporting cultural practice needs and understandings with prospective landlords, property managers*
Helping the person find funding for deposits*
Helping the person organize their move*
Researching possible housing options for the person*
Contacting possible housing options for the person*
Identifying resources to pay for deposits or home goods *
Identifying resources to cover moving expenses*
Completing housing applications on behalf of the service recipient*
Working to expunge records or access reasonable accommodations*
Identifying services and benefits that will support the person with housing instability
Ensuring the new living arrangement is safe for the person and ready for move-in*
Arranging for adaptive house related accommodations required by the person*
Arranging for assistive technology required by the person*
Must have vehicle and insurance. Pass a DHS screening.

Required: 1 year of knowledge with Housing Resources in MN. Bachelor's Degree in human services field or related field preferred; will consider experience in lieu of degree. Must be comfortable driving throughout the metro area. Must be willing to transport clients in your own vehicle. Must be confident using computers programs like outlook. Must reside in Minnesota or within 30 miles of the Twin Cities if in WI.

Now providing Social Security Advocacy Services
Eligibility:
Eligibility Criteria for Social Security Advocacy Services. Individuals served under this must meet all of the following eligibility criteria:

1. If adults, they must have the inability to do any substantial gainful activity by reason of any medically determinable physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than twelve (12) months as described in 20 CFR 416.905 and 20 CRF 404.1505. If children, they must be under the age of eighteen (18), and if they have a medically determinable physical or mental impairment or combination of impairments that causes marked and severe functional limitations, and that can be expected to cause death or that has lasted or can be expected to last for a continuous period of not less than 12 months as described in 20 CFR 416.906; and

2. Live in Minnesota for the duration of the time GRANTEE works with them to apply for Social Security disability benefits; and

3. Individuals must be between the ages of birth and sixty-five (65); and

4. Are at risk for homelessness. An individual must meet one (1) of the following at risk for homelessness definitions:

a. Are behind in rent or utilities,

b. Have an eviction notice,

c. Reside in a boarding house or halfway house,

d. Are exiting a publicly funded institution or system of care such as foster care, a mental health facility, or correctional institution or receive other supportive services in order to maintain their housing,

e. Are receiving state or federal public assistance benefits such as General Assistance, Housing Support, Medical Assistance including Housing Stabilization Services, Supplemental Nutrition Assistance Program, or the Minnesota Family Investment Program,

f. Do not have sufficient resources or support networks immediately available to prevent them from moving to an emergency shelter or place not meant for habitation,

g. Are on a coordinated entry list or housing registry,

h. Are currently a recipient of a housing subsidy program including but not limited to the following: 1. Public housing 2. Housing Support 3. Section 8 Housing or Housing Choice Voucher 4. Veterans Affairs Supportive Housing (VASH) Voucher 5. Moving Home Minnesota demonstration services, also known as Money Follows the Person (MFP) 6. Minnesota’s Long-term Homeless Supportive Services 7. Bridges Rental Assistance Program 8. Family Unification Program (FUP) Voucher 9. Section 811 or other Project-based Rental Assistance Program

OR

5. Are currently experiencing homelessness. An individual must meet one (1) of the following homelessness definitions: a. Lacking a fixed, regular, and adequate nighttime residence such as living in an emergency shelter, couch hoping, living outside, doubled up, living in transitional housing, or places not meant for habitation, b. Not having a lease or ownership interest in a housing unit and lack the resources or support networks to obtain other permanent housing, c. Are fleeing or attempting to flee domestic violence, has no other residence, and lacks the resources or support networks to obtain other permanent housing, d. An unaccompanied youth under twenty five (25) years of age, or families with children and youth who qualify as homeless under other Federal statutes, such as the Runaway and Homeless Youth Act.
