Join us from 9-11 a.m. on Friday, August 28, at 200 San Sebastian View in St. Augustine as we come together to share community updates, learn about local resources and connect with organizations serving St. Johns County. We look forward to collaborating on ways to better support our community.

• CHIPP St. Johns – Learn how the Community Health Improvement Planning Partnership brings community partners together to address community health priorities, strengthen access to care, prevent chronic disease and improve mental and behavioral health throughout St. Johns County.

• H.O.P.E Box – Discover how H.O.P.E Box provides comfort, connection and care to individuals and families navigating grief after su***de loss. Through free, faith-inspired care boxes and thoughtful resources, H.O.P.E Box reminds those experiencing loss that they are supported and not alone.

• St. Gerard Campus – Learn how St. Gerard Campus supports pregnant and parenting young women through its high school, maternity home and Community Care Center. All services are provided at no cost and help young mothers continue their education while accessing resources such as on-site daycare, residential services, diapers, formula, clothing and baby equipment.
• Habitat for Humanity of St. Johns County – Discover how Habitat for Humanity helps local families achieve affordable homeownership through new home construction, home repair and preservation programs and partnerships that create long-term housing stability.

Our team recently attended the Who is Jay Mental Health Symposium, hosted as part of the SMA Healthcare Foundation in Volusia County.

The event brought together more than 350 mental and behavioral health professionals from across the state to promote su***de prevention, mental health education and community awareness.

During the symposium, our team shared information about the BRAVE program and connected with others who are passionate about expanding access to mental health resources and support.

Thank you to the SMA Healthcare Foundation for hosting such an impactful event and for bringing together organizations committed to improving mental health care in our communities.

We invite you to connect with local community partners and learn more about resources available throughout St. Johns County at our next Resource Collective from 9-11 a.m. on Friday, May 29, at 200 San Sebastian View in St. Augustine. No RSVP is required, and all are welcome to attend!

- UF Health St. Johns Care Connect – Learn how Care Connect helps individuals and families access essential health, behavioral health and community support services through resource navigation, care coordination, prevention initiatives and community partnerships. Attendees will also hear about several community-based programs, including the BRAVE initiative, Mobile Outreach Clinic and Youth Leadership St. Johns.

Care Connect is working with a local food pantry to help stock shelves for seniors, veterans and individuals facing food insecurity in St. Johns County.

They are currently in need of donations including:
- Canned vegetables & fruit
- Cereal & oatmeal
- Rice & pasta
- Peanut butter
- Shelf-stable milk & juice
- Toilet paper & hygiene items

Every donation, big or small, helps provide comfort and care to neighbors in need. Thank you for supporting our community. Donations can be taken to the St. John's Food Pantry at 12 Madeore St., St. Augustine, FL, 32084.
