Give it up for The Link Family Resource Center, a member of the Center for Family Strengthening Network, for finding a creative new way to support families! 🎉

The Link recently launched Enhanced Care Management (ECM), giving families:
🤝 A dedicated Lead Care Manager
📋 A personalized care plan
🫶 Coordination across medical, behavioral, school & social services
🔄 Ongoing check-ins and follow-through

What we love about this: ECM is part of California's CalAIM initiative. Meaning The Link found a way to fund and sustain work that looks a lot like what many FRCs are already doing every day. 💡

The Link's experience shows what it can look like to get that incredible work funded and sustained.
