Alcohol Is a Drug. We Just Gave It Better Branding.
How humanity turned a toxic, addictive chemical into a personality, a parenting tool, a health food and the only drug you have to explain not using

Alcohol may be the most successful drug in human history. Not necessarily because it is the most pleasurable, the most addictive or even the easiest to produce, although it can certainly make an argument in all three categories. Alcohol succeeded because it became embedded in human civilization so early and so completely that we stopped treating it like a drug at all.
We drink when babies are born and when people die. We drink at weddings, divorces, promotions, retirements, holidays, sporting events, first dates, last dates, work dinners and backyard cookouts. We drink when we are celebrating, grieving, relaxing, socializing, networking, vacationing, cooking or simply trying to survive Tuesday. Alcohol is somehow appropriate for practically every emotional state a human being can experience. If you are happy, have a drink. If you are sad, have a drink. If you are nervous, have a drink. If you are bored, have a drink. If you are overwhelmed by your children, pour yourself some “mommy juice” and laugh about how parenting is impossible without chemically sedating yourself.
Then we turn around and act horrified that anyone would use a drug to change the way they feel.
That is the part I find fascinating. We have built an entire moral hierarchy of mood-altering chemicals, and alcohol sits near the top, dressed in a nice glass, surrounded by charcuterie and pretending not to belong to the same family. The person who drinks half a bottle of wine every night may still look down on someone who uses ma*****na. The man who gets hammered every weekend may call people taking buprenorphine “drug addicts.” The woman posting “rosé all day” memes may be completely opposed to psychiatric medication because she does not want to put “chemicals” in her body.
Meanwhile, alcohol is ethanol. It is a psychoactive, dependence-producing chemical and a Group 1 carcinogen. It affects nearly every organ system in the body, contributes to injury, violence, cancer, cardiovascular disease and premature death, and kills roughly 178,000 people annually in the United States through excessive alcohol use. Globally, alcohol was responsible for approximately 2.6 million deaths in 2019.
We just happen to sell this particular carcinogenic drug in cute cans and fancy bottles.
We Have Been Drinking for a Very Long Time
Humanity did not recently stumble across alcohol. Evidence of deliberately fermented beverages goes back thousands of years. Chemical analysis of pottery from Jiahu, China, identified a fermented mixture of rice, honey and fruit dating back roughly 9,000 years. Other archaeological findings suggest humans may have been producing primitive beer-like beverages even earlier. Alcohol became intertwined with religion, economics, politics, medicine, hospitality and community in civilizations across Mesopotamia, Egypt, China, Greece and the Americas.
There are practical reasons for some of this. Fermentation preserved calories. Beer and wine could be safer than contaminated water in certain historical settings. Alcohol could be traded, taxed and stored. It also played a ceremonial role and may have helped people bond in larger social groups. A substance that lowers inhibition, eases social anxiety and creates a temporary feeling of connection can be remarkably useful when you are trying to turn unrelated families and tribes into communities.
That does not mean alcohol created civilization, but civilization certainly created a permanent place for alcohol.
Once a substance becomes part of religion, ritual, agriculture, economics and social identity, it becomes harder to evaluate honestly. We are not merely assessing the health effects of ethanol. We are challenging thousands of years of symbolism. Wine is not just alcohol. It is communion, sophistication, romance, adulthood and culture. Beer is not just alcohol. It is football, friendship, masculinity and the reward waiting at the end of physical labor. Champagne is not just alcohol. It is celebration itself.
This is why facts about alcohol so often bounce off people. They are not defending the chemical. They are defending the role that chemical plays in their understanding of normal life.
Why Alcohol Works So Well
Alcohol is appealing because it is pharmacologically messy. It does not act on only one receptor or neurotransmitter system. It alters several systems at once, producing a broad shift in how the brain functions.
One of its most obvious effects is to enhance GABA, the brain’s primary inhibitory neurotransmitter. GABA is basically the braking system. Increasing GABA activity slows neural firing, reduces anxiety, lowers inhibition and produces sedation. Alcohol also suppresses glutamate, particularly activity involving NMDA receptors. Glutamate is the brain’s major excitatory chemical and plays an important role in learning, memory and alertness. By increasing inhibition and reducing excitation, alcohol quiets the brain.
Initially, that quiet can feel wonderful.
The anxious person feels less anxious. The socially uncomfortable person becomes talkative. The stressed person feels relief. The person who cannot stop thinking finally gets a break from their own brain. Alcohol also influences dopamine and the reward system, reinforcing the behavior and making the brain more likely to remember drinking as something worth repeating. It can stimulate endogenous opioid activity as well, adding another layer of pleasure and emotional relief.
Like most addictive drugs, alcohol does two things extremely well: it creates a positive feeling and removes a negative one. People may drink to feel good, but many eventually drink primarily to stop feeling bad.
That distinction matters because alcohol’s early effects can be deceptive. People describe it as a stimulant because they become louder, more animated and more social. Alcohol is not stimulating the parts of the brain responsible for thoughtful decision-making. It is suppressing the frontal systems responsible for restraint. You are not necessarily becoming funnier, sexier or more interesting. Your brain is simply losing its ability to determine that perhaps the story you are screaming at a stranger in the bathroom does not need to be told.
As exposure becomes repetitive, the brain adapts. Because alcohol repeatedly pushes inhibition upward and excitation downward, the brain attempts to restore balance. GABA systems become less responsive, and glutamate systems become more active. That adaptation creates tolerance. The same amount of alcohol no longer produces the same effect, so the person drinks more.
Eventually, the nervous system may become so adapted to alcohol that removing it produces the opposite state: anxiety, tremors, sweating, insomnia, agitation, rapid heart rate, elevated blood pressure, seizures and, in severe cases, delirium tremens. Alcohol withdrawal can kill people. That alone should make it impossible to pretend we are discussing some benign social beverage.
Alcohol addiction follows the same general cycle seen with other addictive substances. The person drinks, experiences reward or relief, and the brain records alcohol as valuable. With repetition, cues associated with drinking begin to trigger anticipation and craving. Meanwhile, the brain’s stress systems become increasingly dysregulated. Alcohol relieves the discomfort it helped create, temporarily restoring the person to something resembling normal. NIAAA describes this progression as a cycle involving intoxication, withdrawal and negative affect, followed by preoccupation and anticipation.
The drug becomes both the cause of the distress and the fastest available solution.
It Does Not Just Damage the Liver
When most people think about alcohol-related disease, they think about cirrhosis. The liver gets all the attention because it performs most alcohol metabolism, but alcohol is not polite enough to destroy only one organ.
Alcohol is absorbed through the stomach and small intestine and distributed throughout the body. The liver converts ethanol to acetaldehyde, another toxic and carcinogenic compound, and then converts acetaldehyde to acetate. During this process, oxidative stress increases, inflammation increases and normal cellular metabolism becomes disrupted.
In the liver, this can progress from fat accumulation to inflammation, fibrosis and cirrhosis. Fatty liver can occur surprisingly quickly and may initially cause no symptoms. People assume normal liver enzymes mean alcohol is not hurting them, but blood work is not a magic window into every stage of organ damage.
In the brain, alcohol affects judgment, coordination, memory, emotional regulation and sleep. Long-term heavy drinking can change brain structure, reduce neuronal size and interfere with networks involved in attention, motivation, decision-making and impulse control. Alcohol can also produce peripheral neuropathy and contribute to thiamine deficiency, potentially resulting in Wernicke encephalopathy and Korsakoff syndrome.
In the cardiovascular system, alcohol is associated with hypertension, atrial fibrillation, cardiomyopathy, heart attack and stroke. “Holiday heart” is not a whimsical Hallmark movie. It refers to cardiac arrhythmias, especially atrial fibrillation, occurring after heavy drinking. Alcohol can weaken the heart muscle, increase blood pressure and disrupt its electrical system.
In the gastrointestinal system, it irritates the lining of the stomach and intestines, contributes to gastritis, reflux, bleeding and impaired nutrient absorption, and alters the gut microbiome. In the pancreas, it can trigger acute or chronic pancreatitis, which is both brutally painful and potentially fatal. It impairs immune function, making the body less effective at responding to infections. It disrupts endocrine function, reproductive health and sexual functioning. It contributes to bone loss, muscle damage and sleep disorders. People frequently use alcohol to fall asleep, but it fragments sleep architecture and reduces sleep quality. You may lose consciousness faster, but unconsciousness and restorative sleep are not the same thing.
Alcohol is also causally linked to multiple cancers, including cancers of the mouth, throat, larynx, esophagus, liver, colon, re**um and female breast. The cancer risk exists because the molecule itself and its metabolite, acetaldehyde, damage cells. The risk does not disappear because the alcohol arrived in a locally sourced pinot noir.
In short, alcohol can seriously f*** you up.
