Alaska Community Care Alaska Community Care provides Care Coordination services to those who choose to stay at home.

Alaska Community Care services include assistance for Older Alaskans, Adults with Physical Disabilities, Individuals with Developmental Delays and Children with Complex Medical Conditions to: obtain and maintain Medicaid; gain access to natural supports, community services and Medicaid Waiver services; initiate and oversee planning process for plans of care; access services including transportati

on, chore, respite, adult day care, supported employment, supported living, in home supports, day habilitation, assisted living/group homes, meals, specialized medical equipment and family habilitation/foster care; face to face visits every month to ensure health needs are met; ongoing eligibility and monitoring service delivery.
