This living document is known as our "Three Year Aging Plan" and will cover 2022-2024. Please note the following update to the free tax preparation program at the ADRC: The ADRC, in partnership with the AARP tax preparation volunteers, are planning to assist folks with filing their taxes and homestead applications this year. The plan is to conduct tax returns by implementing a number of “No Contact” procedures. This will mean additional steps to ensure that everyone remains safe and healthy. On the day of your scheduled appointment please bring your tax documents to the ADRC, a tax preparer will make sure the correct documents are in order. You will then leave the documentation with a tax preparer who will then contact you when your taxes are complete so you can review and sign them.

***At this time, we have started a waitlist. Please call the ADRC at 920.746.2372 to be placed on the waitlist and we will call you to schedule an appointment when a spot opens up.***

You will need the following documents for your appointment:

1. Social Security Card
2. Photo ID
3. If you rent, your Rent Certificate signed by your Landlord
4. Property Tax Bill if you own your home
5. Last years Tax Return

Last year's tax returns are extremely helpful for the tax preparers, so please bring those if you have them available.

If you don't have or forgot any of the first 4 items you will be asked to reschedule your appointment for the next available time.

Cozy winter items such as hats, scarves, mittens, socks, masks, etc. are all needed to fill our "Warming Hearts & Hands" tote. Long-Term Care Options Counseling: If you are not sure about your options for long-term care, contact us for objective, in-depth information about the services and resources available to meet your current and future needs for care. We want you to have all the information you need to make your own well-informed decision. Benefits Counseling: If you have questions about benefit programs, our Disability and Elder Benefit Specialists can talk with you about the benefit programs available and assist you in applying for benefits such as Social Security, Medicare, Medicaid and other publicly funded programs. They can also help you work through problems that arise with your benefits. Health & Wellness: Staying active and healthy is key to maintaining your independence. We can help you make connections with programs that can help support your best possible health and safety. Support for Caregivers: Caring for someone can be a rewarding experience. But, it can also present challenges. We offer information and assistance to help family caregivers care for their loved one and for themselves. Transitioning from Youth to Adult Services: Change can be challenging. We talk with and provide assistance to youth with disabilities and their families as they transition from youth programming to services for adults.
