When families experience housing instability, everything else becomes harder—appointments, medications, school, stress. The SCN partners with housing providers to help Medicaid members get connected to resources faster.

The SCN connects Medicaid members to transportation options that work—whether it’s rideshares, volunteer drivers, or shuttle services.

Even if you’re miles from the nearest clinic, there are ways to get you there.
