Local county health departments are the leading edge of public health services delivery in Oklahoma, responsible for improving public health outcomes for citizens in their communities through strategic disease prevention. Four major service branches, Community Health Services, Family Health Services, Disease & Prevention Services and Protective Health Services, provide technical support & guidance

to all Oklahomans via the Oklahoma State Department of Health & its network of 68 county health departments, as well as guidance & consultation for the two independent city-county health departments in Oklahoma City and Tulsa.

The Oklahoma Rural Health Transformation Program has released a public grant application through a Notice of Funding Opportunity for the Chronic Disease Management Program.

Applications are due by 11:59 p.m. CT on June 8, 2026.

The Chronic Disease Management Program is intended to fund providers, community-based organizations, and other organizations to implement and/or expand proven models for chronic disease management across rural regions of Oklahoma.

Then, they translate this information into recommendations for prevention and control measures.
