Looking for a warm, nutritious meal and great company? Join us at the Victory Baptist Church in Clinton for our congregate dining program! It is a wonderful opportunity to enjoy a healthy lunch while connecting with neighbors in your community. We serve meals every Monday, Wednesday, and Friday at 11:00 a.m. at 133 S. Washington Street. DID YOU KNOW… Thrive West Central is proud to offer the GUIDE Program! This is an initiative supported by Medicare designed to improve the quality of life for those living with dementia or cognitive impairment. The program focuses on enhancing day-to-day living, improving health outcomes and reducing caregiver burden. Participants in GUIDE receive ongoing support from a coordinated care team, including medical oversight from a dedicated GUIDE medical director. A nurse practitioner plays a key role in care delivery, providing annual assessments and dementia symptom management. Thrive West Central is offering the GUIDE program for Indiana residents of Vigo, Parke, Vermillion, Clay, Putnam, and Sullivan counties as well as Clark and Edgar counties in Illinois.
