Position Summary
The Assessment Coordinator is responsible for conducting participant assessments, maintaining accurate consumer records, and coordinating services that support older adults participating in programs at the Tularosa Older American Center. This position ensures compliance with federal, state, and local guidelines under the Older Americans Act (OAA) and Non-Metro Area Agency on Aging requirements.
This is a non-supervisory position that reports directly to the Program Director. The Assessment Coordinator works closely with staff to identify participant needs, document eligibility, and connect seniors with appropriate services including nutrition programs, transportation, and community resources.
Essential Duties and Responsibilities
Participant Assessments
Conduct intake assessments for new participants to determine eligibility for services.
Perform periodic reassessments to monitor participant needs and service utilization.
Maintain accurate and up-to-date consumer records in accordance with program policies and OAA requirements.
Document services provided while maintaining strict confidentiality of participant information.
Case Coordination
Assist in identifying participant needs and coordinate appropriate services within the center.
Monitor participation in congregate meals, home-delivered meals, and transportation services.
Identify participants who may require additional assistance or referrals to community resources.
Coordinate with staff to ensure participants receive services in a timely and effective manner.
Compliance and Documentation
Ensure all assessments and consumer files meet Non-Metro AAA and state reporting requirements.
Maintain organized, complete, and inspection-ready files for monitoring visits and audits.
Assist with compiling data and documentation for program reports and funding agencies.
Participant Support
Serve as a resource for older adults seeking information about services available at the center.
Assist participants in accessing community resources such as healthcare services, benefits programs, and social services.
Promote participation in center programs designed to support independence, health, and well-being.
Program Support
Work collaboratively with the Program Director and staff to support daily center operations and service delivery.
Assist with tracking unduplicated participants and service units as required for reporting purposes.
Participate in outreach activities to inform the community about services available at the center.
Backup Operational Support
Provide assistance with home-delivered meal routes when necessary, including delivery of meals to eligible homebound participants.
Provide transportation services for program participants when needed, including transporting seniors to and from the center or to scheduled appointments in accordance with program guidelines.
Assist with other operational duties as needed to ensure the continuity of services for participants.
Knowledge, Skills, and Abilities
Knowledge of programs serving older adults, particularly those funded under the Older Americans Act (OAA).
Ability to conduct respectful and confidential participant assessments.
Strong organizational, documentation, and record-keeping skills.
Ability to communicate effectively with older adults, staff, and community partners.
Ability to maintain confidentiality and professionalism when handling sensitive participant information.
Minimum Qualifications
High school diploma or equivalent required.
Experience working with older adults, community programs, or social services preferred.
Experience with assessments, documentation, or case coordination preferred.
Valid New Mexico driver's license required for transportation and meal delivery duties.
