📣 Attention Deshler Middle School Athlete Parents! 📣

All parents of student-athletes must create a DragonFly Max account for the upcoming athletic season.

DragonFly Max is the platform used to submit and manage all athletic eligibility documentation, including:

✅ Physical Examination Form
✅ Concussion Information Form
✅ Release Form
✅ Sudden Cardiac Arrest Form
✅ Birth Certificate

Once your account is created, all required forms can be completed and signed electronically. You can also upload a picture of your child’s birth certificate directly through the app.
